For those with insurance, we only accept major PPO commercial insurance plans (Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, and United Healthcare) and Medicare. Although we can check general eligibility for you when you upload your insurance card, the information available is limited. Please make sure you understand the details of your insurance plan, including your copay, co-insurance, deductible, and in-network or out-of-network benefits prior to your visit by contacting your insurance carrier. You will be responsible for any outstanding charges after your insurance plan has paid their portion.
The self-pay fees are as follow:
* Medical Initial Consultation Fee = $200 for the first 15 mins + $100 for each additional 15 minutes time slot
* Medical Follow-up Consultation Fee = $100 for the first 15 mins + $100 for each additional 15 minutes time slot
There is a $35 non-refundable deposit to reserve an appointment slot, and the amount can be applied toward the total visit cost.
Please note: payment is due at the time of service. We only accept patients who are California, Florida, and Texas residents at this time.
